Penn State Federal Platinum Visa™ Credit Card Application

Use this form to apply for a Visa credit card, or to request a new Visa rate or credit increase. Include payroll and tax forms as necessary. Loan Application (DO NOT USE TO APPLY FOR VISA CREDIT CARDS)

Use this form to apply for a loan. Print out and return pages 2-11, and payroll and tax forms as explained on page 1. Fill out and mail in or drop off (we will need your original signature). Do not use this application for Mortgage, Home Equity, Visa, or Home Equity Lines of Credit Loans.

Skip-A-Pay

Stop Payment Request

To request a stop payment on a Penn State Federal check or share draft, use this form. Detach top copy and mail into Penn State Federal. You can also request a stop payment via phone, but must still send in the following form. For questions with Stop Payments, contact Back Office Services.

Net Payroll Deposit Request for PSU Employees

To have your entire PSU payroll deposited at Penn State Federal, complete this form and return it to Penn State Federal, drop it off at your HR office, or mail it to PSU Payroll Department. The PSU Payroll Department address is 307 Ridder I Building, University Park PA 16802. Penn State Federals Routing/transit # is 231386399. PSU requires your original signature. Partial Payroll Deposit Request for PSU Employees

Payroll Cards for PSU HMC Employees

For part of your Hershey Medical Center payroll to be deposited at Penn State Federal, complete this form and mail in or drop off to PSU HMC Payroll Department, Academic Support Building, M.C. A440. PSU HMC requires your original signature. To deposit to a checking account, submit a voided, blank check (not a deposit slip). For additional PSU HMC payroll information, please contact your payroll department.
